As the burden grows, particularly in low- and middle-income countries, the question is no longer if we should act — but how.

Should the next billion dollars go toward preventing disease in the first place — tackling tobacco, air pollution, and unhealthy diets through bold policy and behavior change? Or should the priority be scaling up access to diagnosis and treatment, strengthening primary care systems to support the millions already living with chronic conditions?
